In the password field type
your password (remember that you need to change the password every three months)
or type a full OTP password (activation code + generated password from the card)

If the User name and Password window does not appear, right click
your connection icon and choose Properties.
Click the Option tab
Check the Prompt for name and password
Click OK
Go back to step 2
